Empowering Community Medication Safety: A Five-Year Co-Creation Journey with PharmCare and Drug Icon CC
To ease medication management for patients and caregivers, PharmCare and Drug Icon CC lead social innovation by leveraging design and technology to foster safer practices in communities. Our mission: bridge critical gaps in community pharmacies with intuitive, user-centric solutions that emphasize dispensing safety over traditional clinical workflows. This abstract spotlights our five-year co-creation odyssey—from grassroots needs discovery to AI-driven advancements—showcasing replicable tools for global health equity.

The journey ignited with a profound exploration of unmet needs in native community pharmacies. Conventional systems prioritize medical consultations and backend operations; we, however, crafted frontline dispensing solutions attuned to pharmacists and patients alike. Collaborating with diverse stakeholders—pharmacists, dispensers, patients, and caregivers—we birthed Drug Icon CC: a Creative Commons-licensed visual pictogram system. This innovative labeling tool demystifies drug identification and adherence, tackling persistent barriers like multilingual communication hurdles and verbose educational materials.

Further the engagement HKJC PHARM+ projects in 2024, our solutions has been implemented to 8 more operations with different scale and workflow, this allows us to enhance our solutions to meet different use cases / settings.

Forward-looking, our AI Carebot initiative—co-developed with Health in Action Community Pharmacy—heralds proactive guardianship. This conversational AI, deployed in pharmacies, triages non-clinical queries (e.g., refill scheduling), freeing professionals to prioritize personalized clinical consultations. Early trials show 25% adherence gains, with robust privacy protocols ensuring ethical deployment.

Through this co-creative tapestry, PharmCare and Drug Icon CC illuminate design-thinking's power to democratize medication safety.
